INSTRUCTIONS FOR MITE AND GIRLS 10U – REGISTRATION
OPEN UNTIL 12/31/2011 ONLY PLEASE READ ALL THE WAY TO THE END
Here is the Quakers registration packet you've been waiting for!! Please see the LINKS ABOVE AND the detailed instructions below. You need only print out and complete the packet that applies to your player (either BOYS MITES or GIRLS 10U). PLEASE NOTE: FOR YOUR IMMEDIATE ATTENTION - Please PRINT when completing this form. We need to be able to have your proper contact information. Please see directions for either BOYS MITES or GIRLS 10U – whichever is applies: BOYS MITES - Complete this entire Player Registration Packet – including the DVHL Player/Parent Agreement. The DVHL Agreement should be signed by BOTH the player and the parent. MAIL the completed packet, along with the $500.00 deposit TO: Barbara Larmer Quakers Treasurer 1311 Farren Lane West Chester, PA 19380 GIRLS 10U - Complete this entire Player Registration Packet – including the MAWHA Player/Parent Agreement. The MAWHA Agreement should be signed by BOTH the player and the parent. MAIL the completed packet, along with the $500.00 deposit TO: Barbara Larmer Quakers Treasurer 1311 Farren Lane West Chester, PA 19380 - See below for payment methods. If the Quakers do not receive BOTH the packet and payment, a “spot” will not be held for your player. This is very important to know! PAYMENT METHODS - Payment by Check Checks should be made payable to: Quakers Ice Hockey Club - Payment by Credit Card In the packet you will find the “Quakers Payment Plan” form. If you would like to use a credit card - MasterCard, VISA or Discover – please complete this form and sign each line to authorize EACH transaction. The first payment is the registration fee of $500. To calculate each additional payment, please deduct the $500 from the corresponding fee of the level of your player, and break it up into three (3) payments, so that it is paid in full by August 3, 2011. **REMINDER: If your credit card is stolen, lost, canceled or expires from now until August 3, please notify Barbara Larmer, Quakers Treasurer, immediately at: QuakersMoney@verizon.net Thank you! USA HOCKEY REGISTRATION Beginning April 1, you can register online at www.USAHockey.com for the 2011 – 2012 season. Please PRINT OUT the registration confirmation receipt (with barcode) and bring it to evaluations. Players must have the 2011 – 2012 USA Hockey registration confirmation receipt in order to be out on the ice for practices etc. (Everyone did a great job with this last year- keep up the good work – thank you!) UNIFORMS Uniform information and samples will be available on Wednesday, June 1 - which will coordinate with the June payment date. Please look below for the order form and time. | USA HOCKEY ADM CROSS ICE PROGRAM
Winter - 2011, the Quakers and Ice Line will continue to offer the American Development Model Cross Ice Program from USA Hockey for boys and girls 8U (ages 8 and under).

Helmet, Hockey Pants, Hockey Skates Hockey Gloves, Elbow Pads, Shin Pads, Stick
Rental equipment is available, please inquire when registering. Rentals do not include skates or stick. Skates maybe rented free of charge from Ice Line.

USA HOCKEY REGISTRATION INFORMATION
Online registration for the 2011 - 2012 season will be available starting APRIL 1, 2011.
Follow this Step by Step guide to Online Registration
Individual participants are to follow these steps:
1. Go to www.usahockey.com and click on the “register, online” button. 2. Choose member type Ice Player/Coach. 3. You must be 18 years of age to process a registration…. 4. Check the box that acknowledges that YOU are 18 or older. 5. Follow the prompts…fill in all data correctly. 6. Process payment – Visa, Master Card, Discover or American Express (credit or debit card). A Visa gift card will also work. 7. You may print your Confirmation page at this time. It will also be emailed to you immediately after the registration is completed. You will need to give a copy of your confirmation page to your team manager.
Make sure your members know the following:
Each participant will pay the USA Hockey fee of $30 if age 7 or older.
The USA Hockey fee is waived for age 6 (birth year 2005) and younger -- the registration must still be completed to get the confirmation number.
Participants will also pay any applicable Affiliate fee. Let your members know what the Affiliate fee is so they are prepared.
If you aren’t sure what your Affiliate fee is, contact your Associate Registrar or call USA Hockey 1-800-566-3288 ext 123.
If a participant plays and coaches he/she pays only one registration fee.
Additional information: The home address should be used for all participants. If a player lives in one state and plays in another, enter the home address and pay any applicable
Affiliate fee. The confirmation page/number will be accepted by any USA Hockey program in the country.
Need a computer? Try your local library or school.
A VISA gift card can be used for payment if no credit card is available.
An email address is required. You can use a friend’s address if you don’t have one.
Additional confirmation pages can be requested from the Welcome page of online Registration at www.usahockeyregistration.com Click on the puck in the upper left corner and enter last name, date of birth and zip code. You can then print the page or send it to the email address you enter.
