Team Fund - Projected Expenses Spreadsheet
- This is in .xls format to use for projecting the expenses for the season - Additional ice time, various tournaments, team gatherings, emergency fund etc.  Can be tailored to your team scenario - # of players and season activities.  See additional comments on the file.

Team Manager's Admin Checklist Form - Change the column items as needed.  Some are listed as a reminder for you, since they will need to be collected anyway.  List your player's and coaches.  Check off coordinating box when "item" is collected.  It just helps you to keep track of all the "stuff".  It is in .xls - as a "landscape" spreadsheet.

Wallet Contact Card - This is a template used to create the infamous - extremely helpful - and a great way to promote "communication" among the team - especially to arrange carpools or when traveling.  This is an .xls template - with further instructions right on the file.

Roster Labels - This is all formatted to be used with Avery Labels - which are 2" x 4", clear labels.  Clear labels are much easier to use and line up in accordance with the lines on the roster!  You may only want to print a month's supply at a time, in case there are any changes/updates needed.  DVHL Scoresheets MAY be handled electronically this season - stay tuned!  If not, remember, DVHL scoresheets are a 3 part form - so you'll need 3 labels per game.  Some leagues have scoresheets that are a 4 part form.  It's helpful to give some to the head coach and an assistant coach, so that if you aren't able to make the game, the labels are still there!  Your home scoresheets can be prepped in advance - get a group together and work on it during a practice at the rink.  For MAWHA teams, please refer to your MAWHA Manager's Manual for guidelines.

GIRLS TEAM MANAGERS - MAWHA has created a "Manager's Manual" for all your needs and specifics pertaining to the girls and game requirements, scoresheet procedures and much more useful info.  Please go to www.MAWHA.com - on the left margin, locate the MANAGER'S section and then click on "Manager's Manual".  It's a great resource to print out and have available and is specific to the Girl's procedures.

ITEMS TO COLLECT:

High Performance Athletic Training Center (HPATC) - Informed Consent -  This form must be completed and submitted to your Team Manager prior to your team's participation in their training sessions.  HPATC is located upstairs between Rinks 2 and 3.  It is much easier for the HPATC staff if you collect all the forms at once and then turn them in - prior to their sessions. To see what they are all about, their website is:  www.HighPerformanceATC.com

USA Hockey Registration Confirmation 2011 - 2012 - Since we collected this during evaluations and registration, Managers will no longer need to keep a copy of the player's USA Hockey receipt with them.  However, Managers will need to collect it from ALL coaches - head and assistants - so that it can be submitted to produce your USA Hockey Roster.  Coaches should give a copy of this to you as soon as possible so that you can complete your USA Hockey Roster.  Remind parents to keep a copy of this as USA Hockey will no longer be sending out membership cards. 

Birth Certificate - Parent needs to provide a copy of the player's birth certificate.  Keep the copy of this in your team notebook.

USA Hockey Consent to Treat/Medical Info Form - This is available on this site.  Click on "Team Managers" tab on left - then select "Player Forms" - the link is about 1/2 down the page.

USA Hockey Code of Conduct 2011 - 2012 Form - This is available on this site.  Click on "Team Managers" tab on left - then select "Player Forms" - the link is about 1/2 down the page.  There is a blank left to write in the season (2011-2012).  Please have your players sign and keep a copy in your team notebook.


Coach's CEP Card - Please ask all your coaches to make a copy of both the front AND back of their CEP card (Coaching Card).  Please make sure you can read their CEP# as you may need it for further reference later in the season.  Keep the copy in your team notebook. 
ONLY COACHES ARE PERMITTED ON THE BENCH - WHO HOLD A CURRENT AND LEVEL APPROPRIATE COACHING CARD.

DVHL Code of Conduct-  In lieu of the St. Barnabas Program that was used in the past, parents are asked to complete this one page form for the managers to keep in their team notebook.  This was included in the Quakers Registration Packet and should have been returned with the registration papers.  If not, please click on the file name -   DVHL CODE OF CONDUCT
